Support for Inequalities

The Bridlington Primary Care Network (PCN) provides an outreach service in partnership with the Mind Homeless Hub, offering vital healthcare support for vulnerable patients.

Our service includes:

• Regular health checks and advice for individuals experiencing homelessness
• Follow-up care, including:
  • Contacting patients with blood test results
  • Issuing new medications
  • Arranging follow-up appointments where needed

This proactive approach ensures that some of the most vulnerable members of our community receive continuous, compassionate, and coordinated care.
