Social Prescribing: More Than Just Medicine

As part of the Bridlington PCN Health and Wellbeing team, our Adult Social Prescribing Team is transforming the way we support health and wellbeing in our community. This service goes beyond traditional healthcare by connecting individuals to non-medical support that addresses the wider determinants of our health – such as loneliness, housing, financial challenges, and access to community activities.

The team works collaboratively with GPs, community organisations, mental health services, and voluntary groups to create a tailored approach for each person. By focusing on what matters most to individuals, they empower people to take control of their health and wellbeing in a way that feels personal and sustainable.

The impact of social prescribing is clear: reduced isolation, improved wellbeing, and help people develop stronger community connections. People who have accessed the service report feeling more confident, supported, and engaged in activities that bring meaning and purpose to their lives.
