Supporting the health and wellbeing of children and young people (11-17) is at the heart of our mission – and we’re proud to celebrate the outstanding work delivered by our Children and Young Persons Health & Wellbeing Service, as part of our Health and Wellbeing Team. This service is more than just guidance; it’s about empowering young people and their families to build resilience, confidence, and healthy habits that last a lifetime.
What makes this work exceptional is the collaborative approach at its core. By working closely with schools, CAMHS, social care teams, local community services and our wider PCN Health and Wellbeing Team the service has created a seamless network of support that helps reduce the risk of Young People falling through the gaps. This joined-up approach means families receive early intervention, practical strategies, and emotional support when they need it most.
From helping Young People manage anxiety and improve school engagement to supporting parents with tools to navigate challenging times, the service has transformed lives in ways that ripple across the community. Professionals and families alike praise its ability to listen, understand, and tailor solutions to each child’s unique needs.
